8/15/2023 EnchantaVaughn Approved Behavior: As the session began, JaCarei took a few minutes to gather himself before going to talk to QMHP-C he his mother reports she just got on him for throwing items in the house. QMHP-C informed JaCarei this session was going to focus on him learning how to control his anger and express his feelings in an appropriate manner. Behavior: JaCarei displayed in appropriate age-appropriate skills and impulsive, hyperactive behaviors evident by him running from room to room during the session. Intervention: QMHP-C will meet with JaCarei daily to discuss progress and expectations that are working and not working in the home weekly. QMHP-C inquired of JaCareisâ current mood. QMHP-C utilized active listening skills as JaCarei spoke of his current mood. QMHP-C utilized active listening as information was conveyed. QMHP-C will provide JaCarei with support and encouragement. QMHP-C will continue to build rapport. QMHP-C communicated with JaCarei on displaying appropriate control. Intervention: QMHP-C reflected with JaCarei on his day, reviewing both good and bad experiences. QMHP-C provided a summary of the session, to reassure JaCarei had clear knowledge of the purpose of session. QMHP-C directed JaCarei to verbally express positive affirmations. QMHP-C will explore positive self-talk, use instruction modeling, and increase social communication skills with positive reinforcement role play activity. QMHP-C will educate JaCarei on behavior modification to help JaCarei identify what behavior he needs to decrease and avoid daily. Response: JaCarei was able to reevaluate himself and display a more positive attitude toward peers and authority. JaCarei was observed having positive interactions with everyone in the home. JaCarei demonstrated responsibility and leadership by helping his mom around the house. JaCarei implemented his plan for restoring relationships, and this was reviewed. JaCarei demonstrated expectations and responded well to feedback. JaCarei continued to interact in a positive manner with his mother and counselor. Response: JaCarei took his mind from thinking and doing to a place of concentration, which allowed him to focus on one thing at a time. JaCarei nodded in agreement. JaCarei was observed displaying age appropriate behaviors and was productive and studious throughout the day. JaCarei demonstrated clear understanding of objectives. JaCarei expressed positive self-talk, modeled positive behavior, and good communication skills during role play activity.
